Relief Team Leader


Job description

The Team Leader is part of the support team whose function is to meet the practical, physical and emotional needs of service users within the service. Through a key worker system, they will also be responsible for the designing of individual programmes of support and coordinating the delivery of the package by a group of Support Workers and themselves..

Responsibilities

  • To plan co-ordinate and implement the safe transition of new Service Users into service and community where necessary.
  • To liaise with Service User families and relevant members of multi-disciplinary teams, community groups and voluntary bodies with the aim of promoting the Service User needs.
  • To plan, develop and implement individual and group programmes of support to enhance and maximise the capabilities and independence of the Service Users in the Service.
  • To co-ordinate ongoing assessment and review on a regular basis through the completion of suitable assessment models.
  • To support the physical, emotional and personal needs of Service Users ensuring that work is Service User focused and upholds the principles of respect, privacy, dignity, fulfilment, independence and choice.
  • To provide effective leadership, supervision and management to Support Workers, including monitoring of time and attendance and the participation in any capability, disciplinary and grievances procedures etc.

Skills and Qualifications

Essential Skills

  • 2 Years paid experience in a Health and Social Care setting
  • Or Understanding of the mental health and/or learning disability field plus one of the following qualifications.
  • All applicants must be registered, or eligible to register at the time of appointment, on the appropriate part of their professional register (NISCC, NMC, HCPC).
  • Or Level 5 Diploma in Leadership for Health Social Care (Adults Management / Adults Residential Management) (Wales and Northern Ireland) and 2 years paid employment experience in a health and social care setting
  • OR QCF Level 3 in Health and Social Care and 2 years paid employment experience in a health and social care setting
  • Time management skills
  • Full valid driving licence on application and access to own transport on appointment. (If a disability precludes an individual from holding a drivers licence, it should be stated how they can meet the mobility requirement of the post)
  • Proof of eligibility for permanent employment with Praxis Care in Northern Ireland at time of application
  • Professional qualification in Social Work,
  • Professional Qualification in Nursing
  • Allied Health Profession qualification
  • Degree in Psychology and 1 year paid employment experience in a health and social care setting
  • Communication skills both verbal & written

  • Computer skills
  • Available to work flexibly
