Practice Nurse


Job description

The Practice Nurse will work as part of the primary care team to provide nursing services to the practice population by evidence based, quality focused care. To assess, plan, implement and evaluate care by demonstrating safe clinical decision making. To ensure clear channels of communication exist between clinical, administrative and community teams and to project a positive and friendly image to patients and other health care professionals..

Responsibilities

  • Deliver expert care for patients within general practice
  • Make professional autonomous decisions.
  • Help the practice to develop and deliver specialist primary nurse led service.
  • Provide assessment, treatment, screening and health education services and advice, working in accordance with set protocols, guidelines and NMC Code of Conduct.
  • Provide nursing service/ treatments to patients independently or in participation with general practitioners with reference to agreed protocols and guidelines.
  • Assist in and perform routine tasks related to patient care as directed by the GP Partners.
  • Perform annual health checks for patients with various health conditions and comorbidities.
  • Chronic disease management and prevention
  • Carrying out and completing care plan reviews
  • New patient medicals
  • Venipuncture
  • Measure patients observations. Such as BP, Pulse, Resps, O2 sats, complexion, height, weight and temperature
  • 24 hour BP machine fitting and management of results
  • Cervical cytology
  • Asthma/ peak flow readings/ spirometry
  • Hypertension management
  • Vaccine/ injection administration
  • Childhood immunisations
  • Child protection/ looked after child awareness
  • Healthy lifestyle advice, including diet, exercise, alcohol and drug advice
  • Smoking cessation advice
  • DMARD monitoring
  • Well women/ HRT/ Contraception check-ups/ breast examination advice
  • Well man check-ups/ testicular examination advice
  • Handling and requesting pathology tests. Such as urine, stool, sputum and swabs
  • Process referrals within your clinical scope
  • Chaperoning and assisting patients where appropriate, who are being examined by another clinician
  • Following agreed clinical protocols
  • Support and co-ordinate health promotion campaigns
  • Maintaining high levels of infection control and keeping up to date with any changes in guidelines
  • Maintaining and cleaning equipment
  • Maintaining GP and nurses rooms, stocking and rotating equipment
  • Vaccine and fridge stock management, maintaining the cold chain
  • Ensure safe storage, rotation and disposal of vaccines and medicines
  • Participation in administrative systems in the practice
  • Maintain accurate patient records at all times
  • Maintain appropriate clinical coding in accordance with QOF guidelines
  • Maintain effective liaison with other agencies and staff concerned about a patient
  • Follow and adhere to all aspects of confidentiality protocols
  • A duty to advise the practice manager of any problems or errors which may occur
  • Attend and participate in any practice meetings
  • Any other delegated duties appropriate to the post
  • The role of the Practice Nurse is an evolving one and it is anticipated that the post holder will be responsible for being reasonably flexible in the interpretation of the above job description.

Skills and Qualifications

Essential Skills

  • Degree, Diploma or other nursing qualification allowing registration with NMC
  • 5 years experience of working within a nursing team
  • Experience of multi-professional working

Desirable Skills

  • Advanced certificate in Independent and Supplementary Nurse Prescribing
  • Qualified and registered to complete cytology
  • Qualification in asthma management
  • Qualification in diabetes management
  • Experience of working in a GP practice
  • Experience of using Vision/ Docman IT systems
