Job description

The post holder will work as part of a Multidisciplinary Team based in Mental Health Services. He She will ensure that safe and effective care is planned and delivered to clients. The post holder will be expected to make decisions about complex and professional issues and to work in conjunction with other professional colleagues to ensure appropriate intervention and care for clients..

Responsibilities

  • The practitioner will work with the CRHTT Team Manager in developing and delivering on an integrated Mental Health Service Trust wide promoting effective interdisciplinary and interagency relationships.
  • Practitioners will accept referrals and provide timely response and assessment via single point of referral system/ Triage as per operational agreed standards.

Skills and Qualifications

Essential Skills

  • Applicants must have 5 years post qualifying experience as: Registered Nurse whose name is entered on Part 1 (Mental Health) of the register maintained by the NMC OR A registered Social Worker with the Northern Ireland Social Care Council.
  • 3 Years experience as a Band 6 in an Acute Mental Health setting, which must include experience of working within mental health unscheduled care.
