Job description

The post holder will provide continuity in the care and support of patients with heart failure and their families through the provision of Nurse Led outpatient clinics and in-patient reviews. They will liaise with primary and secondary care to provide seamless service. .

Responsibilities

• To review in-patients and participate in discharge planning and establish a system to follow up patients with heart failure.
• Maybe required to arrange home visit or further contact with the patient post discharge within reasonable time frame.
• To plan, implement and evaluate programmes of care for patients with heart failure using current guidelines for practice and treatment including nurse prescribing.

Skills and Qualifications

Essential Skills

• Registered Nurse, Adult branch and live on NMC register.
• Have a minimum of 5 years post registration experience working in an acute cardiology setting to include at least one year as band 6 or above.
• Have NMC recorded Specialist Practice Qualification in Cardiology, equivalent, or willingness to undertake.
