BHSCT Patient Pathway Co-ordinator
Job description
Proactively tracks the progress of urgent patients along their pathway from point of referral to diagnosis, treatment and discharge. This will include the co-ordination of reports, X-rays/investigation results, Multidisciplinary Team Meetings (MDT) discussion and clinic appointments to expedite the patient’s diagnosis and treatment..
Responsibilities
- Proactively track all urgent patients and take appropriate action to ensure a timely diagnosis and treatment for these patients, as required to achieve clinical and Department of Health targets.
- Support the flow of information to and from other departments and Trusts including responding to queries.
- Responsible for ensuring all patients on urgent vascular pathways have pre-booked appointments and treatment is for ensuring all patients on urgent vascular pathways have pre-booked appointments and treatment is in line with the clinical pathways.
Skills and Qualifications
Essential Skills
- 4 GCSEs Grade C or above, to include English (excluding English Literature) and Mathematics or equivalent qualifications and 2 years administrative experience (at Band 3) OR
- HNC/HND or equivalent / higher qualification preferable in an administrative related field and 1 years experience (at Band 3) in a clerical /administrative role OR Three years administrative experience at Band 3 AND
- Experience of Medical Terminology OR OCR Stage 2 Text processing (Part 1) and Stage 2 Word processing or medical audio transcription.
- Working experience of the Patient Administrative System (PAS) AND other Hospital Information Systems.
Desirable Skills
- Knowledge of Vascular Services. Use of PAS, NIECR and Imaging Systems.
