Patient Flow Coordinator / Clinical Coordinator Band 7
Job description
Work as part of a team which facilitates the patient journey through the acute hospital setting. Under the direction of the Lead Nurse they will be responsible for the efficient and appropriate utilisation of beds and are required to balance the demands of emergency and elective clinical activity. Effectively coordinate clinical team work activities meeting the patient need, carry out clinical tasks within their sphere of competence and appropriately refer for further clinical assessment of care. Ensure that they and all nursing staff adhere to NMC Codes, Trust policies and Procedures, and work in a professional and courteous manner to promote the Trust’s values. Be part of the total Site Manager/Clinical Coordinator rota and will be expected to rotate to fulfil this Rota. Liaise with the Clinical leads/ HoS, AD and Director on call and provide reports for the Director of Acute services as required. Function as part of the Hospital @ night team and will support the junior medical staff and the ward nursing staff by providing clinical leadership, having extended clinical skills and a broad knowledge base to promote safe, effective and timely patient centred care. In their span of duty will direct the Medical Assistants in their duty rota’s and any leave issues. Carry out any other duties as directed by Head of Service for Patient Flow..

- Provide leadership to nursing staff,they will be a role model demonstrating patient centred care and work in partnership with Ward Sisters/Charge Nurses to promote and monitor safe and effective practice enhancing the patient experience.
- Contributing to the delivery of the Trusts objectives.
- Co-operate fully with the implementation of the Trust's Health and Safety arrangements.
- The post holder will be the Senior Nurse on site in the OOH period, at weekends and Bank Holidays.
Skills and Qualifications
Essential Skills
- Currently a Registered Nurse qualified on the live NMC register.
- Experience of operating computer based systems.
Desirable Skills
- Experience of managing a group of staff.
- Experience of working in an acute setting.
