Job description

To provide a highly responsive, individualised information and signposting service to people with a diagnosis of dementia which supports people living with dementia and their carers to continue to live as independently as possible within their local community, with an improved sense of well-being and control over their day to day life. The Navigator will map out and connect the individual with dementia and their carer with statutory services including GPs, voluntary, community and independent providers. Responsibilities

• To market the Navigator Service and develop a network of support from health and care professionals who will refer people with a diagnosis to the service at the earliest opportunity
• To ensure that barriers for those who find services difficult to access are actively minimised, providing a welcoming service
• To provide structured support including an initial referral appointment and follow up as required throughout the trajectory of the illness
• To co-develop with the person with dementia and their immediate carers, families and supporters an agreed 'information plan'

Skills and Qualifications

Essential Skills

• A professional qualification in nursing, social work or an allied health profession and live on the relevant register Or Honours degree in psychology and appropriate postgraduate applied psychology qualification
• And Three years post registration/qualification relevant experience And 2 years working in a health facility or setting Or 1 year working in a community health team setting
• And Evidence of further professional development/attendance at courses not mandatory since registration

Desirable Skills

• 2 years working in a mental health facility or setting or 1 year working in a community mental health team setting in older peoples mental health

Further Information

Salary: £41,659 - £47,672 per annum pro rata.
