Job description

The Clinical Nurse Facilitator will work as part of the Care Home Support Team (CHST) to provide training, advice and support to Care Home staff to promote high quality, coordinated, person-centred care. Please see employer website for full job details & person specification. .

Responsibilities

• Provide palliative care and tissue viability consultancy and provide recommendations for the development of treatment plans for identified needs
• Facilitate Care Home staff to gain knowledge, competency and confidence in clinical aspects of nursing care, highlighting the importance of professional accountability
• Improve care outcomes for people in Care Homes

Skills and Qualifications

Essential Skills

• Hold a Nursing qualification and on Part 1 of the NMC Register
• Minimum of 3 years post registration nursing experience Or 2 years post registration experience with relevant Palliative Care and/or Tissue Viability Module

Desirable Skills

• Post Graduate modules in Palliative Care
