Community Nurse Palliative Care


Job description

Foyle Hospice has immediate vacancies for Community Palliative Care Nurses to deliver expert end of life care to those with complex palliative/end of life needs in their own homes and care homes. The successful candidates will be registered Nurses on Part 1 (Adult) of the live NMC Register. Three year’s post-registration experience caring for patients with cancer/palliative care needs in a health care setting is essential, as is an accredited module/course in palliative care e.g. European Certificate/CEC module. Community experience is desirable .

Responsibilities

  • Develop, facilitate and instigate a philosophy of care and approach to care which is holistic and individualised, by ensuring that assessment of patient needs is carried out and that individual care programmes are implemented and evaluated
  • Assist in the development of written standards of care in partnership with the multidisciplinary team to conduct audits of standards in order to evaluate effectiveness of care delivery
  • Work as an integral part of the multi-professional specialist palliative care team to ensure palliative care practice is evidence-based throughout the Hospice
  • Establish effective working relationships with the Specialist Palliative Care Team in the WHSCT, long-term conditions, General Practitioners and District Nursing teams
  • Assist in the development of Care Pathways for this group of patients
  • Work in collaboration with existing providers to educate care professionals in specialist palliative care at both basic and post-basic levels.
  • Ensure that collaboration between community and Hospital services is seamless, effective and efficient

Skills and Qualifications

Essential Skills

  • Level 1 Registered Nurse (Adult) on the NMC Live Register
  • An accredited module/course in Palliative Care e.g. European Certificate/CEC module
  • Relevant three years post registration experience at Band 5 or above in Cancer, Palliative Care in a health care setting
  • Clinical care of patients with palliative care needs. Demonstrate knowledge of clinical governance, research and audit
  • Evidence of dealing with complex and difficult emotional situations
  • Teaching and facilitation skills
  • Evidence of effective communication and multi-professional working
  • Car driver with full up to date driving licence

Community Palliative Care Nurses (Band 6, Agenda for Change pay scale) Current posts available: 34 hours permanent, 4 days per week and 17 hours permanent, 2 days per week.
