WHSCT_Hospital Flow Patient Tracker, Band 4
Job description
The post will require a person who is highly motivated, has high levels of initiative, is methodical and organised and who can work under pressure and who has excellent communication skills. The Patient Tracker should take an active role in supporting the site manager and Patient Flow in ensuring the patients journey to discharge is seamless and any delays or issues identified are escalated via the site meetings.
Responsibilities
- Ensure all equipment and screens are powered on with information required for the 08:30 site status meetings in the Site Coordination Hub.
- Record potential and confirmed discharges per ward as specified in the morning site status meeting.
- Visit wards with patients identified for discharge and ensure that FLOW boards are updated to accurately reflect patient status. This will involve actively updating the FLOW system at ward level / adding electronic notes to the system if required.
Skills and Qualifications
Essential Skills
- HNC/HND or equivalent/higher qualification in an administrative related field within a health care setting with direct patient contact; OR
- 5 GCSEs at GRADE A-C including English language and Maths or equivalent / higher; AND 2 years experience in a clerical /administrative role within a health care setting with direct patient contact.
