Job description

Reviewing the needs of all clients with dementia/mental health diagnosis in residential and nursing home facilities on a systematic basis to ensure the appropriateness of the service, at all times seeking the views of clients and carers, where appropriate. Functioning as a named worker supporting clients in permanent care and their carers through the care management process. Responsibilities

• To implement relevant legislation, Department and Trust Policies and Procedure
• Assess the nursing care needs of clients who self-fund their nursing home care to determine their eligibility for the nursing care payment under the HPSS Payments for Nursing Care Scheme
• Managing an allocated workload and establishing priorities within it in the interest of good time management and delivery of an effective and efficient service
