Mental Health Community Keyworker
Job description
The Keyworker will work as a member of the Integrated Primary Care Multi-disciplinary Team/Mental Health Services for Older People Team to screen, co-ordinate, assess and identify services to meet the needs of vulnerable Older People in the community who are most at risk and who require support to enable them to remain at home. The Keyworker will be responsible for the provision of high quality deprivation of liberty safeguards (DoLS) and Mental Capacity Act interventions to service users and their families. The Keyworker will work in partnership with individuals in a person centred approach, to assess need and agree outcomes by co-production with the Service User, carer to deliver a service. The Keyworker will also undertake the role of Investigating Officer, where appropriate, as part of the Adult Safeguarding Policy and Procedures. They will ensure Safety, Quality and patient experience is central to the ethos of their practice and to that of the team and mirrors the Trusts corporate objectives..
Responsibilities
- The Keyworker will receive and screen referrals for the assessment of peoples health and well-being needs. The Keyworker will work closely with community based and voluntary organisations to meet the identified assessed need
- The Keyworker will assess need and develop a person centred support plan for each service user based on their individual assessment.
Skills and Qualifications
Essential Skills
- Have a minimum of two years post qualifying experience, working in a statutory community or ward setting with adults with a mental illness, including dementia.
- Have a minimum of two years recent experience in a paid or voluntary basis working with adults who lack capacity.
