Job description

The post holder is accountable and responsible for the coordination of the admission and discharge of cardiology patients to and from the Regional Cardiology Centre. The post holder will be a key person in communication between multi-disciplinary team members, patients, families, external care providers and other Trusts to ensure the patient is admitted and discharged safely to the right place, at the right time. The post holder will be accountable and responsible for resolving complex issues relating to the admission and discharge process of cardiology patients and working closely with the Cath Lab scheduling team and Primary PCI Coordinator to ensure timely admission/discharge of our inter-hospital transfer patients..

Responsibilities
