Lead Pharmacist Community Mental Health (with on call)


Job description

Develop Mental Health Medicines Management (MHMM) services in the community sector of the Trust. Lead on the development of Mental Health Medicines Optimisation programme. Co-ordinate care from secondary into primary care, working closely alongside local community mental health teams (CMHT) and charities. Include the development of specialised medicines management support for mental health patients and include medication review, monitoring and adherence support. Support the implementation of Trusts Medicines Management policies and procedures within the community sector of the Trust..

Responsibilities

  • Lead the development of Mental Health Medicines Management (MHMM) in the community sector of the South Eastern HSC Trust, in collaboration with the Consultant Psychiatrists and the Regional Clinical Pharmacy Development Leads.
  • Develop the work to meet any changing needs and contribute to strategic planning.

Skills and Qualifications

Essential Skills

  • 5 years post registration experience including 3 years in hospital of which 2 years must be at band 7 or above
  • MPharm or equivalent and eligible to register with PSNI

Desirable Skills

  • Experience in mental health clinical pharmacy.
  • Qualified as a Pharmacist Independent Prescriber.
  • Post graduate qualification in a mental health/psychiatric related subject area or be enrolled in such a course.
