Job description

The post holder will lead, deliver, develop, evaluate and be responsible for delivery of pharmacy homecare medicines services ensuring appropriate clinical and governance arrangements, in accordance with the Royal Pharmaceutical Society (RPS) standards for homecare services. The post holder will be responsible for the development of the Trust homecare medicines policy and will be responsible for review and continuous improvement of homecare medicines services in conjunction with the Homecare Medicines Committee.

Responsibilities

• Responsible for the development of pharmacy homecare medicines service.
• To contribute effectively to the care of homecare patients through the appropriate and optimal use of medicines with limited side effects.
• Provide advice on processes associated with homecare medicines management to patients, carers and homecare contractors.
• Be an Independent Pharmacist Prescriber.
• To demonstrate and be able to apply expert clinical knowledge and an advanced level of clinical reasoning and judgement.

Skills and Qualifications

Essential Skills

• MPharm in Pharmacy or equivalent.
• Registered with the Pharmaceutical Society of Northern Ireland (PSNI) or eligible to register.
• Registered/eligible to register as a Pharmacist Independent Prescriber or willing to undertake.
• Completed or enrolled in a Post Graduate Masters, or equivalent, in relevant area of pharmacy.
• 3 years in hospital, 2 of which must be at Band 7 or above.
• 2 years post registration experience working as a clinical pharmacist.
• Experience leading a clinical pharmacy service.

Further Information

Salary: £53,755 - £60,504 per annum pro rata. Location: Portadown - Craigavon Area Hospital, 68 Lurgan Road, BT63 5QQ
