Job description

The Community Discharge Service aims to speed up hospital discharge times and to improve outcomes for older people and adults requiring support. This Community Discharge model aims to assess for and deliver care in a more appropriate setting and improve the experience of patients who no longer need the care of an acute hospital but are able to manage at home with support or in an interim care home setting. The post holder will visit clients at home or within an interim placement facility, who have been recently discharged from hospital, within the agreed timeframe, to complete an occupational therapy assessment. Following this assessment the post holder will either make recommendation for any community based services for example reablement /community rehabilitation or complete short term and time limited interventions that will enable people to gain or regain their confidence, ability, and necessary skills to live as independently as possible. The post holder will contribute specialist Occupational Therapy clinical skills and knowledge to the Community Discharge Service. These skills will involve assessment of all aspects of occupational performance (physical, cognitive, mental, social and environmental needs) and use clinical reasoning, experiential knowledge, and clinical skills to deliver a client-centred service. In response to continued service development the post holder will be required to undertake flexible working including extended working and seven day working as determined by operational requirements.

Responsibilities

  • To manage a caseload of clients in the community who have been identified as requiring the community discharge service.
  • To carry out manual handling/ risk assessments and implement subsequent actions.

Skills and Qualifications

Essential Skills

  • Degree/Diploma in Occupational Therapy from a school recognised by the World Federation of Occupational Therapists (WFOT) Be registered with the Health & Care Professions Council. (HCPC)
