Care Manager
Job description
The Care Manager will be responsible to the Learning Disability Service Manager for the commissioning and delivery of care for adults with learning disability living in the community or who are currently in Muckamore Abbey Hospital. The Care Manager will work closely with service users’, carers and families alongside other professionals, both hospital and community, to identify needs with a view to developing and implementing the most appropriate care plan for service users. The Care Manager will provide ongoing monitoring and review of care plans that include provision of domiciliary care, supported living, residential and nursing home placements. The Care Manager will be expected to work within a busy and dynamic team, assessing risk and managing same within their caseload, and participate in the office Duty system accordingly. The Care Manager will be expected to act as an Investigating Officer under the Adult Protection Policy for clients they are keyworker for.
Responsibilities
- Advocate on behalf of clients with a learning disability and their families.
- Convene and Chair, when necessary, multi-disciplinary case discussions to consider risk and legislative issues.

Skills and Qualifications
Essential Skills
- A recognised professional qualification in Social Work; Nursing; or any of the professions allied to medicine, with a minimum of 3 years post qualifying experience.
