Senior Practice Nurse (Band 7)
Job description
The Practice Nurse will be a specialist resource providing clinical expertise in care, advice and support to the nursing team within the practice area. The Practice Nurse will operationally manage the workload of the professional nursing staff, effectively managing staff rotas, appointment systems and working in collaboration with GP colleagues and other professional staff.
Responsibilities
- The Practice Nurse will provide leadership and direction for professional nursing staff within the Practice to promote the delivery of safe and effective care.
- The Practice Nurse will lead and be responsible for the delivery of safe and effective practice for self and the team whilst contributing to the delivery of the Trusts objectives.
- The Practice Nurse will be required to work alongside multidisciplinary colleagues in order to ensure a seamless, coordinated approach to person-centred care, across the clinical pathway, timely onward referral to other professionals and agencies.
- The Practice Nurse will lead in the provision of expert advice in developing, implementing and monitoring policies, procedures and protocols which meet national, regional and local requirements.
Skills and Qualifications
Essential Skills
- Registered Adult Nurse: first level, on the live NMC register.
- 3 years’ experience at Band 5 or above in a Primary Care environment.
- Have a Post Graduate Diploma or Degree in Chronic Disease Management Or willing to undertake and successfully complete relevant modules within 2 years of appointment to the role in order to be able to practice independently.
- Hold a current full driving licence which is valid for use in the UK and have access to a car is essential. This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved.
Desirable Skills
- Nurse Prescribing qualification.
