Job description

Provide nursing care to a person with learning disability and complex health needs in the adult centre. Care within the home may also be required. He/she will work as a member of a Multi-Disciplinary Team providing specialist Learning Disability nursing support, advice and practical assistance as required.

Responsibilities

• The post holder is responsible for the assessment of care needs and the development of programmes of care and/or the implementation and evaluation of these programmes of care. The post holder is expected to carry out all relevant forms of care.
• The assessment, planning, implementation and evaluation of patient care.
• Carry out evidence based practice and agreed care pathways.

Skills and Qualifications

Essential Skills

• Registered Nurse on Part 1 of the NMC Register.
• At least 6 months paid experience of working as qualified nurse in the United Kingdom.
• In addition to the above if your NMC has lapsed and if you have not been employed in a nursing position within the last 5 years you must have completed a Return to Nursing course.
• Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment. This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport.
