Mental Health Nurse
Job description
The post holder will work within the Community Mental Health Services for Older People (CMHSOP) multi-disciplinary team, providing skilled assessments and evidence based interventions in a community setting to older people with mental health problems, and/or their carers. The post holder will contribute to the establishment of systems and processes to ensure effective communication and continuity of client care, liaising with multi-disciplinary / multi-agency teams and community services. They will ensure Safety, Quality and patient experience is central to the ethos of their practice and to that of the team and mirrors the Trusts corporate objectives. The post holder will promote and support effective team working, fostering a culture of openness and transparency. The post holder will ensure that they take all concerns raised with them seriously and act in accordance with the South Eastern Health & Social Care Trust’s Raising Concerns Policy and their professional code of conduct, where applicable.
Responsibilities
- Provide guidance information and support to professionals and other carers who offer care to people with dementia / functional mental illness, which is highly developed specialist knowledge and is underpinned by specialist training and experience.
- Have responsibility for the management of own caseload by admitting, managing and discharging within agreed Protocols / Guidelines and by offering a variety of highly specialised treatment interventions and discharge planning options.
- Act upon referrals from General Practitioners, other Primary Care professionals and the Psychiatric sector teams, providing assessment, implementation and development of care programmes to individuals, families and groups where dementia / functional.

Skills and Qualifications
Essential Skills
- Two years post-qualifying experience in Adult Mental Health.
- Registered Mental Health Nurse, live on the NMC Register.
- Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.
