Job description

Transitions supports young disabled people explore options and next steps as a school leaver. The service offers specialist support to individuals so that they can make decisions about their future and personal development goals with confidence. The service supports young people and their families during this transitional period.

Responsibilities

  • To deliver the Transition Service fully involving young people and their families to make decisions about their life after school.
  • The Transition Officer will use an inter-agency approach to ensure a smooth transition post school into education, employment as well as exploring health & well-being opportunities in their local area.

Skills and Qualifications

Essential Skills

  • RQF level 6 (e.g., Bachelor’s Degree) or equivalent level professional qualification and a minimum of 1 years’ paid experience in the direct provision of education, training or support
  • Or Minimum RQF 4 or equivalent in a relevant discipline and a minimum of 3 years’ paid experience as above.
  • Proficiency in IT skills, specifically Microsoft Word, Excel, Outlook.
  • Good communication skills.

Further Information

Salary- Starting on £29,540 per annum (pro-rata). Belfast Trust, Ravenhill Reach.
