Job description

The AC@HT primary focus is on maintaining older people at home in the event of a crisis or unexpected deterioration in health. It aims to reduce length of stay for inpatient admissions, prevent hospital admissions and facilitate early discharge. The Rehabilitation Health Care Assistant will carry out ongoing delegated nursing / therapy support to patients who require ongoing support, using a patient centred approach. The duties to be carried out with each patient will be clearly specified by the nurse / physiotherapist / occupational therapist and will reflect the assessed need of the patient, enabling him / her to maintain as much independence as possible. The post holder will be required to work at times without direct supervision. During the training period the post holder will be required to develop competence in the duties outlined below. Responsibilities

  • Work as an integral member of the multi-disciplinary team, carrying out duties as agreed and delegated by the relevant professional using a patient centred approach to the delivery of rehabilitation.
  • Assist in maintaining a safe environment both in the office/department and the community setting in which care is being delivered
  • Provide assistance with rehabilitation as outlined in the patient’s care plan and agreed with the relevant health care professional (nurse, physiotherapist, occupational therapist).

Skills and Qualifications

Essential Skills

  • NVQ 3 in a Care related subject
  • Or NVQ 2 in a Care related subject
  • Or 4 GCSEs at Grades A-C including English Language or equivalent / higher qualification and 1 years’ experience in a health/social care related role
  • Or 2 years’ experience in a caring role within a health / social care related role

Desirable Skills

  • Experience working within a rehabilitation setting
