Job description

The post holder is required to assist in the provision of safe effective compassionate patient centred care in patients own homes and community clinics. The post holder will assist the Multidisciplinary Diabetes Team to carry out reviews and care of patients with diabetes in the Community. The post holder will undertake physiological observations/clinical duties as required in clinics and patients homes. The post will involve 7 day cover to include day, weekends and bank holidays to meet the needs of the service as required. Responsibilities

• Undertake patient assessment in the community when delegated by Diabetes Specialist nurse in adherence with BHSCT protocols and work within own level of competency under supervision of Diabetes Specialist Nurse.
• Be responsible for observation and reporting back findings on the patient’s condition to the responsible Diabetes Specialist Nurse including any concerns or changes to delegating Diabetes Specialist Nurse.

Skills and Qualifications

Essential Skills

• Hold NVQ/VRQ/QCF level 3 Certificate in Health/Health & Social Care or an equivalent qualification, which includes diabetes training within qualification obtained.
• Have 2 years’ experience of delivering direct care in a care environment.
