Job description

To work in partnership with the multi-disciplinary team and in collaboration with the young people and their families, to provide the highest possible standard of care for the young people undertake allocated duties of care under the supervision of the Community Learning Disability Nurse Manager. The post holder will have continuing responsibility for the assessment, development, implementation and evaluation of plans of care for young people in the Community who have complex health needs, severe learning and/or physical disability. Responsibilities

• Assessment, Planning provision and evaluation of effective nursing intervention and adapting it to meet the changing needs of the individual whilst encompassing and supporting the physical and emotional health and wellbeing of children with a sever.
• Acting as a nurse within the multi-disciplinary team to ensure that the nursing needs of children with a severe learning disability in the community are met, through the use of various clinical, technical and other therapeutic activities.

Skills and Qualifications

Essential Skills

• Currently a Registered Nurse Learning Disability on Part 1 on the Live NMC Register.
• Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.
