Job description

In order to meet the overall mission of the organization the post holder will: Be a highly specialist resource providing clinical expertise in care, advice and support within the specialist area of Diabetes Lead and or work within the multi disciplinary team in order to ensure a seamless, co ordinated approach to person centred care, across the clinical pathway, and timely onward referral to other professionals and agencies as appropriate. Lead and or participate in the provision of expert advice in developing, implementing and monitoring policies, procedures and protocols that meet national, regional and local requirements. Proactively advance the modernization agenda of the Diabetes Service for patients and the development of the nursing service to achieve person centred care. Contribute to advancing the body of knowledge through education, research, service improvement and audit as well as developing practice in others Services will be provided 7 days per week, 52 weeks of the year, staffed on a Rota basis Please visit www.jobs.hscni.net to see Job Description and Personnel Specification.

Responsibilities

• Act as a specialist expert resource in the planning and communicating complex / highly complex programmes of care to all disciplines and in partnership patients / clients, families and their carers, to support timely access to relevant care.

Skills and Qualifications

Essential Skills

• Registered Nurse on Part 1, Adult Nursing, of the NMC Register
• Access to a car or form of transport that will enable the post holder to carry out duties that involve meetings with community stakeholders.
