Job description
The jobholder will be responsible for the management and performance of the nursing staff in the Hospital at Home Service. Promotion of the Hospital at Home Service and Geriatric outpatient services. Monitoring the provision of care provided by the Hospital at Home Service. Measuring the outcome of patient care. Responsibilities
- Responsible for managing the performance of the Hospital at Home Service and ensuring robust processes are in place to support the delivery of high quality and safe care to meet all requisite standards.
- Ensure there are robust access and care pathways to support the co-ordination of person centred care and integrated working.
- Ensure the effective implementation of a case management approach with all key worker and team roles clearly understood.
- Effectively plan manage the use of the nursing resource within the Hospital at Home through the completion of monthly off-duty rota, delegation of tasks on a day to day basis and oversight of bank staff usage.
Skills and Qualifications
Essential Skills
- Hold a professional qualification in Nursing.
- Be a currently registered nurse Level 1 - Adult on Live NMC Register.
- 2 years post graduate experience at a senior level i.e. Band 6 or above.
- Clinical experience working within a setting involving the care of acutely unwell elderly patients.
