Job description

Post holder will participate as a full member of the multidisciplinary mental health team for older people and will have responsibility for the assessment of care needs, development, implementation and evaluation of programs of care and ensuring their application at all times for both service users with functional mental illness conditions and with a diagnosis of Dementia presenting with complex issues. He/she will provide formal professional supervision and teaching for more junior grades of nursing staff including students who report directly to him/her. Responsibilities

• Undertake comprehensive assessment of service users with enduring mental health illness and/or dementia.
• Plan, implement and evaluate individual care plans with involvement of all parties concerned in the care of the service user.

Skills and Qualifications

Essential Skills

• Level 1 Registered Nurse (Mental Health) on the live NMC register.
• 2 years post registration experience within a mental health setting.
