Job description

The Referral Coordinator will support our referral process from pre-entry to post-exit. They will be responsible for processing referrals in a professional, sensitive, and timely manner in line with MACS procedures and processes underpinned by MACS policies and operating in line with our strategic priorities, vision, mission, and values. Responsibilities

  • First point of contact for receiving referrals for all MACS Floating Support and Outreach services.
  • Manage the referral process and waiting list for MACS Floating Support and Outreach services.
  • Complete all referral administration, such as recording and storing data accurately and securely, in line with data protection legislation.
  • To provide a supportive, calm, and professional approach while interacting with referrals to our Floating Support and Outreach services via various communication avenues.
  • Proactively progress waiting lists for our services in line with operational procedures. Maintain contact and support with young people on waiting list for MACS Floating support and Outreach services.
  • Highlight any concerns with regards to risk for individual referral pathways with line manager for guidance and decision-making.
  • Support onward referral process for young people being externally signposted to other specialist services.
  • Compliance with organisational safeguarding and risk management procedures.

Skills and Qualifications

Essential Skills

  • 3 years’ experience in a health and social care setting supporting referral processes/appointment setting.
  • Computer literate with working experience of using a variety of software, in particular Microsoft packages.
  • Ability to demonstrate excellent verbal and written communication skills in both professional and personal interactions.

Desirable Skills

  • Knowledge and understanding of the services available to people with lived experience of homelessness.
  • Experience working in voluntary sector.

Further Information

Salary: £27,803 per annum pro rata.
