Job description

The Band 6 Community Mental Health Nurse will work in any Mental Health Team as part of the overall delivery of Mental Health services within the South Eastern Health & Social Care Trust. Mental Health Services are delivered twenty-four hours a day, seven days a week, including Bank Holidays, in both hospital and community environments and the postholder will be expected to work flexibly to ensure the delivery of the service. The Practitioner will work within a matrix organisation promoting effective inter-disciplinary and inter-agency relationships and acting as a link between clients, professionals and other services to ensure continuity of care. As a member of the multi-disciplinary team, the Practitioner will provide skilled assessments and provide evidenced based interventions in a variety of settings to people with mental health problems, and their carers. The successful applicant will be required to work in any part of the South Eastern Health and Social Care Trust area within any mental health team or service as needs dictate. Responsibilities

  • To accept appropriate referrals, undertake assessment of individuals' mental health and social needs and identify potential risks (KSF HWB2).
  • To assess health and wellbeing needs and strengths and develop, monitor and review care plans to meet specific needs.

Skills and Qualifications

Essential Skills

  • Two years post-qualifying experience in Adult Mental Health.
  • Registered Mental Health Nurse, live on the NMC Register.
