Community Psychiatric Nurse


Job description

The post holder will be a member of the multidisciplinary Community Rehabilitation Team (CMHRT) providing multi-disciplinary holistic assessment, treatment, interventions and support to individuals with complex mental health needs. In collaboration with Service Users / Clients, the CMHRT will assist the individual to identify recovery-orientated goals and provide a range of specialist rehabilitation interventions with focus on addressing and minimising symptoms and functional impairment. The aim is to reduce supports and inputs as the individual reaches, achieves these goals, and obtains optimum levels of independence and recovery. In addition the post holder will undertake the role of Keyworker including the management of a defined caseload as an autonomous practitioner with responsibility for mental health assessments, risk assessments, risk management plans, care reviews including contingency and crisis management plans, care planning and the implementation of care plans involving carers where appropriate. Responsibilities

  • To assist with the development of the Community Rehabilitation Team and be responsible for prioritisation, organisation and management of own caseload and workload.
  • To be responsible for assessment, treatment and management of clients including formation of treatment and rehabilitation plans, providing appropriate intervention and delivery of service.
  • To demonstrate a detailed knowledge of the presenting conditions and apply a high level of understanding of the effect of illness/disability and the impact on functional performance.

Skills and Qualifications

Essential Skills

  • 2 years post registration experience to include at least 12 months relevant speciality experience within a mental health setting.
  • RMN Qualification and be currently registered on part 1 of the NMC Live Register (Mental Health).
  • Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment.
  • This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved by the Trust which will permit them to carry out the duties of the post.
