Advanced Care Planning Nurse (Band 6)
Job description
The main purpose of this role is to be an expert practitioner with highly specialised renal knowledge to develop the delivery of supportive and palliative care to patients with advanced kidney disease. The candidate will be responsible for providing a high quality holistic palliative care service to renal patients both in the hospital and in the community. The post holder in collaboration with the multidisciplinary team will be directly responsible for assessing, planning, implementing and evaluating patient care and act autonomously, using a person-centred approach within the his/her scope of clinical practice guided by the NMC Code. Responsibilities
- The postholder will oversee all aspects of renal supportive care in conjunction with the multi-disciplinary team.
- Assess, review and manage patients in end stage renal failure who have decided not to progress towards a renal replacement therapy, including visiting the patient at home where appropriate and providing specialist input to formal clinic.
- The post holder will demonstrate comprehensive skills for assessment, review and support for patients currently on dialysis who are experiencing increased symptom burden. He/She will assess and help advise on treatment and develop programs of care.
- They will continue to keep knowledge and skills up to date by engaging in a range of relevant learning and development activities.
- The post holder in collaboration with the multidisciplinary team will address and deliver targets for end of life care.
Skills and Qualifications
Essential Skills
- Currently a Registered Nurse (Adult) on the live NMC Register.
- You must hold the full Renal Course post graduate qualification or ENB 136 or equivalent.
- You must hold or be willing to undertake additional qualifications reflecting an interest in palliative care or a suitable alternative provided by the clinical education centre or other appropriate body.
- 3 years post registration experience working in a renal setting excluding time spent in Post Registration Training.
- Hold a full current driving licence and have on appointment access to a car. This will be waived in the case of applicants who are prevented from driving due to a disability, providing the applicant can organise suitable transport.
