Job description

To work as a member of the multi-disciplinary diabetes team, providing a service to people with diabetes in the Southern Health & Social Care Trust area. The post holder will develop and provide a quality service to patients, carers, health & social care professionals and other agencies. The postholder will be expected to support Community Diabetes services Trust wide.

Responsibilities

• To plan and prioritise own work, ensuring safe caseload management.
• To work closely as part of the multi-disciplinary team to ensure an integrated and quality service.

Skills and Qualifications

Essential Skills

• B.Sc./Diploma in Nutrition and Dietetics or equivalent recognised professional qualification.
• Post graduate qualification in Diabetes or be willing to undertake this if required.
