Job description

The Hospital at Home team primary focus is on maintaining older people at home in the event of a crisis or unexpected deterioration in health. It aims to reduce length of stay for inpatient admissions, prevent hospital admissions and facilitate early discharge. The Health Care Assistant will carry out ongoing delegated nursing / therapy support to patients who require ongoing support, using a patient centred approach. The duties to be carried out with each patient will be clearly specified by the nurse / physiotherapist / occupational therapist and will reflect the assessed need of the patient, enabling him / her to maintain as much independence as possible. The post holder will be required to work at times without direct supervision.

Responsibilities

• Work as an integral member of the multi-disciplinary team, carrying out duties as agreed and delegated by the relevant professional using a patient centred approach to the delivery of rehabilitation.
• Assist in maintaining a safe environment both in the office/department and the community setting in which care is being delivered
• Assist the trained members of the nursing team in the delivery of a high standard of nursing care within the community

Skills and Qualifications

Essential Skills

• Hold a current full driving licence which is valid for use in the UK and have access to a car on appointment
• Have 2 years’ experience of delivering direct care in a care environment
