Job description

The post holder will be expected to work across primary and secondary care sites as an expert in this clinical field under the direction of the Band 7 Specialist Nurse, to deliver specialist clinical diabetes services to patients and their families/carers for both inpatient care and outpatient care, will contribute to the strategic working and will be required to influence the development of the service both locally and regionally.

Responsibilities

• Manage a defined caseload, work autonomously to achieve person centred care.
• Communicate complex, sensitive information effectively and empathetically to patients and their families.
• Maintain diabetes helpline by telephone, email, text etc.
• Assess, plan, implement and evaluate holistic specialist diabetes nursing care in inpatient and outpatient setting.
• Act as a source of specialist advice for medical and nursing teams on the management of diabetes while in hospital.

Skills and Qualifications

Essential Skills

• Be a Registered Nurse on Part 1 of the NMC live Register.
• Evidence of continuous post-registration personal training and development.
• Hold a specialist Practice Qualification in Diabetes or willing to undertake a specialist practice qualification in Diabetes.
• Minimum of 3 years’ post registration experience, 2 years’ of which, excluding courses, must have been in providing Diabetes care to adults.

Desirable Skills

• Knowledge of structured patient education.
