Job description

To have continuing responsibilities for the home therapy population within their defined areas. To organise and run clinics for transplant/home therapy/non-inpatient/pre-dialysis patients. Responsible for the ongoing training and care of patient opting for home dialysis. Have on-going responsibility for teaching of staff, patients and relatives and create a good learning environment. To deputise for the Manager in their absence. To cover for other members of the team, as required. To rotate within renal services, as required by service needs

Responsibilities

• Ensuring the individual care needs of all non-in-patient/pre-dialysis patients are assessed by a qualified nurse.

Skills and Qualifications

Essential Skills

• Registered on Part 1 of the NMC Register with 5 years post-registration experience.
