Project Worker
Job description
Provide the highest possible standard of care and support to Service Users by actively promoting and implementing Recovery by using a strengths based person centred approach. Please see employer website for full job details & person specification.
Responsibilities
- Key working sessions, planning and reviewing care, and helping to maintain tenancies.
- Manage medication, liaise with health teams, facilitate group activities, and champion wellbeing.
- Support Service Users to manage their Mental health, physical health and wellbeing through the development, implementation and monitoring of individual wellbeing and safety management plans.
Skills and Qualifications
Essential Skills
- To be flexible as the role will involve working on a rota/shift basis in Supported Housing. Shifts will include early shifts, late shifts, night shifts, weekend and/or night cover as required to meet the needs of the scheme.
- Be educated to QCF/NVQ Level 2, or equivalent, in a related subject e.g. Care and 6 months paid experience supporting people within a health and social care setting.
- Or A minimum of 12 months paid experience supporting people within a health and social care setting.
- Have proven experience of motivating, supporting and mentoring Service Users.
- Experience of maintaining service user and agency records.
- Working knowledge of using IT systems.
