Job description
The H@H service primary focus is on maintaining older people at home in the event of a crisis or unexpected deterioration in health. It aims to reduce length of stay for inpatient admissions, prevent hospital admissions and facilitate early discharge. The post holder will function fully as a member of the H@H Multi-disciplinary Team and will be required to participate in all multi-disciplinary meetings, share decision making, advise on specific Nursing requirements and complete patient held and shared files for all patients and carers in a person centred manner. Responsibilities
- To provide the delivery of high level care to individuals, demonstrating professional competence in assessment, decision making and the management of clinical needs.
- Work in partnership with the multi-disciplinary team and GP colleagues to provide nursing skills that would enable people to remain in their own home.
- Provide care for people with undifferentiated or undiagnosed problems to prevent admission or readmission to care settings.
- Work as a member of the multi-disciplinary team and will co-ordinate the care for people on the team’s case load.
- To make recommendations for changes to practice within speciality area. This may lead to the implementation of specific changes to practice or contribute to the development of service protocols.
Skills and Qualifications
Essential Skills
- Registered Nurse Qualification and currently registered on Part 1 (Adult) of the NMC Live Register.
- Applicants must have 2 years’ post-registration experience and able to evidence knowledge of Care of Elderly.
- Hold a full current driving license valid for use in the UK and have access to a car on appointment.
- This criterion will be waived in the case of a suitable applicant who has a disability which prohibits them from driving but who is able to organise suitable alternative arrangements in order to meet the requirements of the post in full.
Desirable Skills
- Computer literate.
- Experience of being involved in implementing evidence based practice.
- Evidence of Post Basic Education.
