Job description

The jobholder will assist the Integrated Care Team Manager with the implementation and delivery of an integrated model of care for one of seven geographical teams. They will work very closely with social work, physiotherapy and occupational therapy lead colleagues to ensure there is a co-ordinated and integrated approach to the assessment, delivery and monitoring of high quality and safe care. As the Professional Lead District Nurse the jobholder will be responsible for the delivery of high quality district nursing services and the day to day supervision of the team’s nursing staff. The jobholder will ensure that all nursing practices comply with the requisite professional standards and NMC Code of Conduct. The jobholder will also assist the Integrated Care Team Manager with service planning, financial and resource management.

Skills and Qualifications

Essential Skills

• 5 years Post-Registration Nursing experience, 2 years Post Specialist Practice Qualification.

Desirable Skills

• Experience of working within an Integrated Care Team setting.
• 2 Years line management experience.
• Leadership skills.
