Clinical Pharmacist, Falls Prevention Service with on call Band 7


Job description

In order to meet the overall mission of the Trust the post holder will: Deliver a Falls Prevention Clinical Pharmacy Service, incorporating medicines optimisation and evidence-based fracture-risk assessment to support falls prevention within the Community Falls Prevention and Management Service to provide an enhanced level of care post-fall and improve patient outcomes. Accountable for carrying out fracture-risk assessments to identify patients at risk of fracture and prescribe bone-strengthening medicines as per national guidelines (NICE, NOGG, FRAX). Independently refer appropriately assessed patients to consultant geriatricians for radiological screening for osteoporosis and osteopenia. Work as a Clinical Pharmacist in the Community Falls Prevention and Management Service providing a medicines review and optimisation service, utilising independent prescribing skills, in order to identify any prescribed medications that may contribute to the risk of falls. Responsibilities

  • To practice as an independent prescriber providing individualised, evidence-based medicines optimisation, clinical reviews and fracture-risk assessment of older patients who are at risk of falls as part of the trust Falls Prevention and Management.
  • Demonstrate and apply advanced clinical knowledge, clinical reasoning and judgement when making recommendations to optimise medication and bone health treatment plans to patients with complex medical needs.
  • Provide a high level of clinical pharmaceutical care, as part of the Falls Prevention Team responsible for the follow up of older patients (65 years and over) and to screen and assess these individuals for modifiable falls risk factors...

Skills and Qualifications

Essential Skills

  • Minimum of 2 years post registration experience of which 18 months (by 31/08/2026) must be as a band 6 hospital pharmacist Or Minimum of 2 years (by 31/08/2026) post - registration experience in General Practice pharmacy.
  • MPharm or equivalent.
  • Member of the Pharmaceutical Society of Northern Ireland or eligible for membership.

Desirable Skills

  • Experience providing pharmaceutical care to older people with comorbidities and polypharmacy.
  • Experience within General Practice Pharmacy of running or participating in a clinic to provide pharmaceutical care.
