Job description

The post holder will work as a member of the Children’s Disability Team in WH&SCT. The post holder will carry continuing responsibility for a defined caseload of children with nursing needs, and will be responsible for the assessment planning implementation and evaluation of programmes of care. The post holder will have an in-depth knowledge of the Nursing needs of the children with a Learning Disability. Working within a multi-disciplinary team the post holder will be responsible for assessment of needs, risk assessment and management, development of Nursing care plans, medication monitoring, regular review in keeping with WH&SCT and regional policy procedures and guidelines. Within this role they will be responsible for delivering nursing skills and impart learning disability knowledge within a multi-disciplinary team framework. Responsibilities

  • To undertake comprehensive and holistic health assessments of people with learning disabilities with diverse and complex presentations, using investigative and analytical skills to formulate individualised care and treatment plans
  • Using clinical reasoning and utilising a wide range of skills and options to formulate a specialised programme of care to ensure health is preserved and/or improved.
  • To manage referred caseload of children with nursing needs, and participate and contribute to comprehensive and holistic/health assessments.
  • Manage ongoing assessment planning implementation evaluation of nursing care plans, risk assessments and management, therapeutic interventions, medication monitoring and any arising identified needs ensuring multidisciplinary team working.

Skills and Qualifications

Essential Skills

  • Registered nurse Level 1 (Adults, Mental Health, Learning Disability or Children’s) on the live NMC register.
  • 2 years post qualifying experience which must be working with individuals with Learning Disabilities.
  • Hold a current full driving licence which is valid for use in the UK and have access to a car is essential.
  • This criteria will be waived in the case of applicants whose disability prohibits driving but who have access to a form of transport approved by the Trust which will permit them to carry out the duties of the post.

Desirable Skills

  • At least one year post qualifying experience of working therapeutically with children and young people with a learning disability in a community setting.
  • Community Learning Disability Qualification.

Further Information

Location: Lilac Villa Gransha Park. Salary: £38,682 - £46,580 per annum.
