Job description
Aging well project has been developed as a proactive model of care aiming to keep people well at home and avoiding hospital admissions. The project is one of a number of improvement initiatives aligned to the minister’s reset plan, rethinking models of care delivery towards a more preventative and anticipatory led service delivery. The project will deliver multi-professional and inter-agency approaches to early identification and management of frailty syndromes in people over 65 years of age. The project team will consist of Occupational Therapy, Physiotherapy, Nursing and Social Care. The design of the project is to offer a home visit to people over the age of 65 who are not yet known to the health service, however, presenting with symptoms of early frailty. The post holder will work collaboratively to deliver advanced Nursing clinical skills and knowledge to support the assessment, early intervention, prevention and reversal of frailty within the community setting. The post holder, as an advanced practitioner, will use evidence-based/ client-centered principles to assess, plan, implement and evaluate Nursing interventions. This will include the provision advice, education and training of clients, carers, family and onward referral/sign posting to other health care groups as appropriate.

Skills and Qualifications
Essential Skills
- Registered Nurse on Part 1 (Adult)) of the live NMC Register.
Desirable Skills
- Evidence of participation in audit and/or research or, Evidence of involvement in Service Improvement initiatives and outcomes.
- Specialist knowledge of Older people.
