Community Navigator


Job description

We are currently recruiting for a Community Navigator to join our Mental Health & Addiction Services team. This is an exciting and innovative role focused on supporting people to build stronger social connections, increase community engagement and reduce loneliness. Working directly with service users, you will support individuals to identify meaningful activities, reconnect with their communities, and build confidence in developing and maintaining social relationships. This role forms part of a nationally funded research study, contributing to the development of new approaches to improving social inclusion and wellbeing.

Responsibilities

  • Develop & maintain a trusting & professional relationship with Service Users and act in a supportive & empathetic manner towards them.
  • To identify local resources, opportunities and activities that can enhance social contact and reduce loneliness for mental health service users.
  • To deliver one to one focused support to a small case load using a community navigation model.
  • To model personal responsibility, self-awareness, self-belief, self-advocacy and hopefulness.
  • To actively support the research study through which these roles have been developed, and to contribute to development of the intervention, record keeping and providing feedback to study researchers as required.

Skills and Qualifications

Essential Skills

  • To be flexible in order to meet the needs of the Community Navigator Role.
  • Be educated to QCF/NVQ Level 2, or equivalent, in a related subject e.g. Care and 6 months paid experience supporting people with mental health issues within a health and social care setting. OR
  • A minimum of 12 months paid experience supporting people with mental health issues within a health and social care setting.
  • Have proven experience of motivating, supporting and mentoring Service Users.
  • Have proven experience of supporting individuals with a range of mental health diagnosis.
  • Experience of maintaining service user and agency records.
  • Working knowledge of using IT systems.
