Job description

The Support Worker will be responsible for delivering practical and emotional support to individuals whose illness impacts on their day to day living. They will work as part of the Community Mental Health Team. This team offers a service for adults who have mental health needs. The team offers time-limited and long-term interventions to individuals depending on assessment of need. The support worker will focus on enabling service users to exercise safe and positive life choices and to develop skills and resources to aid their recovery and independence.

Responsibilities

• To provide advice, support and assistance where necessary to help service users achieve the short and long-term goals as indicated in recovery plans.
• To support service users in all areas indicated by their recovery plan using a recovery focused approach designed to maximise their potential and promote their independence.
• To implement agreed individual programmes of support and activity designed to enhance and maximise the independence of service users (as agreed with service users, carers, families and the multi- disciplinary team.

• To actively encourage social, occupational and recreational plans as relevant to individual service users.

Skills and Qualifications

Essential Skills

• QCF/NVQ/OCN Level 3 in Health and Social Care (or equivalent) OR QCF/NVQ /OCN Level 2 in Health and Social Care (or equivalent) and 1 years’ experience, paid or voluntary working in a health/social care setting, providing direct support/care.
• Currently registered or be eligible for registration with NISCC.
