Team Lead Lead Speech and Language Therapist; Community Stroke. This post holder will support the delivery of a specialist Speech and language therapy service as an integral member of the Community Stroke Rehabilitation team (CST). The post holder will independently manage and provide a highly specialist speech and language service to adults with acquired communication and associated swallowing difficulties in community settings. The band 7 will be required to operationally manage the patient flow against expected targets for the Early Supported Discharge (ESD) and Community Rehabilitation service, supporting the wider team in dealing with the range of problems that may be experienced by patients in the community setting. The post holder will work in partnership with the Community Stroke Service manager to operationally manage the ESD/CST service. Responsibilities

• To operationally manage all referrals to the CST/ESD, jointly with other team leads, delegating to appropriate MDT members ensuring assessments are completed within targeted timeframes
• Liaise closely with patients/carers and provide specialist advice and support regarding client’s speech, language, communication, eating and drinking difficulties to ensure best outcomes

Skills and Qualifications

Essential Skills

• 3 year post graduate experience working as an SLT with adults with acquired disorders of communication and swallowing in an acute setting at Band 6 level or equivalent
• Evidence of on-going CPD to include post-graduate, informal or in-service training in adult acquired communication and swallowing disorders

Desirable Skills

• Evidence of relevant postgraduate training, specialist short courses to Masters equivalent level
