Job description
The purpose of Discharge coordinator is to facilitate safe discharge for patients who have been declared medically fit by the medical team. The post holder will be responsible for coordinating complex discharges from the respiratory wards. This involves working with the hospital multi-disciplinary team and Community Respiratory Team and early supported discharge. On discharge, the post-holder will be required to oversee and coordinate the multi-disciplinary assessments for service users, to ensure they achieve maximum outcomes by undergoing a period of further assessment rehabilitation. Responsibilities
- To operationally manage a Respiratory Discharge Service and act as a source of expertise and knowledge to facilitate safe and effective discharge in conjunction with the clinical team.
- Maintain effective communication with medical, nursing staff, patients, family/carers and other members of the multi-disciplinary team related to admission/discharge planning.
Skills and Qualifications
Essential Skills
- A relevant University Degree or a professional qualification with current registration with the appropriate regulatory body (e.g. NMC, HCPC).
- A minimum of 5 years post-registration experience, including at least 3 years’ experience at Band 6 level within a clinical and/or managerial role.
