Senior Social Work Practitioner


Job description

The post holder will be based within the SHSCT’s Integrated Care teams (ICT). The post holder will be responsible for the direct provision of high quality Social Work interventions to services users and their families. The post holder will be expected to provide support, professional advice and supervision to social workers, social care workers and other colleagues as appropriate. The post holder will work very closely with other colleagues to ensure there is a co-ordinated and integrated approach to case finding, screening, allocation, assessment, delivery and monitoring of high quality and safe care. The post holder responsibilities will include the ability to demonstrate enhanced level of practice, competence and skills in areas such as direct practice with individuals and families, multiagency and multi- disciplinary working, Service user and Carer involvement and demonstrate knowledge of organisational systems and processes and the wider social policy context. The post holder will carry a designated social work caseload from across a number of teams simultaneously and will ensure that all social work and social care practices comply with the requisite professional standards and NISCC Code of Practice for Social worker. The post holder will also hold a range of complex Adult Safeguarding responsibilities and will also undertake the role of Designated Adult Protection Officer (DAPO) and Investigating Officer (IO) as required. The post holder will be based within a peripatetic model and will provide a Trust Wide service.

Responsibilities

  • To provide a professional Social Work service to Older People and manage a complex caseload working in accordance with the legislation and SHSCT policies and procedures.
  • To undertake assessment, intervention, commissioning and case management for Service Users and Carers.
  • To understand the impact of caring on Carers and to refer for Carers’ assessments as identified under the Carers and Direct Payments Act (Northern Ireland) 2002, including the provision of Carers’ Assessments.

Skills and Qualifications

Essential Skills

  • Hold a professional Social Work qualification (CQSW, CSS, DipSW, Degree in SW).
  • Have NISCC registration or be eligible for immediate registration or have lodged an application with NISCC.
