Job description

The Learning Disability Liaison Nursing Service Band 6 role has been developed to ensure that the needs of adults with a Learning Disability are met when they require admission to Hospital, either Acute or Mental Health. The Liaison Nurse is a member of the Adult Learning Disability Community Nursing Team based within the general hospital setting. They will work across Trust supporting the Acute and Mental Health Hospitals/Wards. To manage appropriate referral caseload and contribute to the clinical systems of the wider multi-disciplinary team as a whole. Promote and monitor safe and effective practice. Adhere to Trust Values. Responsibilities

  • Act as a resource and a point of contact for Acute/ hospital staff, carers, patients and other professionals and stakeholders, to support the delivery of prompt, safe and effective care for patients with learning disabilities.
  • Accept patient notifications / referrals, refer on as appropriate and plan and instigate discharge from care under the direction of the Band 7
  • Work effectively both individually and as part of multidisciplinary teams.

Skills and Qualifications

Essential Skills

  • Registered Learning Disability Nurse Part 1 on the live NMC Register
  • AND, Have two years' post-registration experience within learning disability nursing setting
  • Demonstrate the knowledge and ability to provide effective leadership and clinical skills to meet the needs of the post in full.
