Post title: Family Support and Engagement Coordinator Responsible to: Project Manager Location: Gasyard Centre and outreach locations across Derry/Londonderry
Responsibilities
- Service and Leadership:
- Lead and further develop the effective delivery of innovative family support services for families affected by cancer, aligned with Hive’s strategic vision and business plan.
- Plan, design, deliver and evaluate programmes and activities for families, ensuring they are evidence based, person centred and responsive to identified needs.
- Develop and implement clear referrals, assessment and review pathways for families, ensuring timely support and appropriate onward referrals e.g. clinical teams, mental health, physical health, programmes and activities, community and voluntary, etc
- Ensure that family support services are all inclusive, accessible and culturally sensitive, with particular attention to families experiencing disadvantages, complex needs or bereavement.
- Family Engagement and Case Coordination:
- Act as a primary point of contact for families, building trusting relationships and providing high quality emotional, practical and informational support.
- Undertake and oversee initial assessments, home visits and follow up contacts, using agreed person centred planning and risk assessment tools.
- Co produce individual support plans with families, setting clear goals and regularly reviewing progress with them.
- Identify and signpost or refer families to appropriate internal and external services, ensuring effective follow up and feedback.
- Facilitate group based support for parents, children and young people where appropriate e.g. peer support, psycho educational sessions, activity based programmes, in collaboration with colleagues and partners.
- Safeguarding, Quality and Governance:
- Ensure best practice in safeguarding children and vulnerable adults, acting as a safeguarding lead within the family support function and following Hive’s policies and relevant legislation.
- Maintain up to date knowledge of relevant legislation, guidance and service developments in family support, cancer support, and mental health, and incorporate these into practice.
- Implement and contribute to quality assurance systems, performance indicators and outcome measurement tools, supporting continuous improvement of services.
- Ensure accurate, timely and confidential recording of all casework and family contacts on agreed information systems, in line with data protection and organisational policies.
Skills and Qualifications
Essential Skills
- Relevant degree in a health, social or community field
- Or 3 plus years direct family support experience
- And 3 plus years in community work and 2 plus years supervising staff or volunteers.
Further Information
Salary NJC Scale 4, spinal column point 11 NICVA guidance, pro rata. The current full time equivalent for SCP 11 is set in line with the NJC pay agreement and updated annually the pro rated salary for 30 hours
