SEEDS Project Worker


Job description

The SEEDS Project Worker will support the setup, implementation and roll out of the DEEDS social model of care in Causeway area to include the delivery of programmes and services for older adults living with dementia, support for their carers and work towards improving Dementia awareness across the wider community. Working in partnership with the Healthy Living Centre Alliance SEEDS Project lead and the SEEDS Regional Manager the post-holder will provide frontline support, facilitate activities, and encourage engagement to help people living with Dementia and carers to improve wellbeing, maintain independence, reduce isolation, and remain connected within their communities. The role requires a compassionate, person-centred approach alongside strong partnership working, community engagement, and organisational skills.

Responsibilities

  • Work as part of the Causeway Rural & Urban Network team to support the delivery of the organisation’s strategic priorities and objectives.
  • Promote the values, vision and aims of the organisation.
  • Programme Delivery and Facilitation
  • Support for PWD and Carers Provide respectful, person-centred support to older PWD and carers participating in the programme.
  • Organisational Responsibilities.

Skills and Qualifications

Essential Skills

  • A third level qualification in a health social sciences subject AND one year’s experience working with vulnerable adults including people living with Dementia OR Three years’ experience working with vulnerable adults.
  • Engaging vulnerable people including those living with dementia, their families and carers in a range of activities, programmes or services.
  • Working in partnership with community, voluntary and statutory agencies to support older people.
  • Design and delivery of programmes and services promoting independent living, healthy lifestyles, social, physical and emotional wellbeing.
  • Facilitation and group work with individuals with a diagnosis of dementia.
