Job description

This is a unique opportunity to make a real difference, helping young people live safely and independently within their community, with 24/7 tailored support to promote stability, resilience, and personal growth. This role is varied and rewarding, supporting young people through challenges such as mental health struggles, substance misuse, housing stability, education, and building positive futures.

Responsibilities

• Ensure the needs of service users are effectively met through promoting independence through support, development and progression.
• Provide support and advice to service users.
• Undertake an assessment of a service users’ needs and risk to ascertain intervention level required.
• Develop focused outcome-based support plans and risk management plans and monitor delivery.
• Liaise with external agencies where appropriate, to ensure co-ordination and integration of services, and maximise opportunities.
• Act as an advocate, providing support to and on behalf of clients; attend appeals or tribunals; liaise with GPs, other healthcare professionals, statutory and voluntary agencies.

Skills and Qualifications

Essential Skills

• Experience of supporting Looked After Children or homeless and/or vulnerable adults to identify personal goals and meet person-centered targets while working through a process of change.
• Experience of working within a fast moving and challenging working environment in the areas of mental health, care for young people, alcohol and drug use, independent living support.
• Experience of using a high level of communication skills, both verbally and in writing, using Microsoft Office applications.
• Full, current driving licence and regular access to a suitable vehicle for work-related travel.
• NISCC Registered (or willing to become registered if appointed) and willing to be subject to Access NI checks. (Successful applicants must commence NISCC registration process prior to employment).

Desirable Skills

• At least 6 months' experience supporting young people (aged under 25) who have experienced social exclusion, disadvantage, or involvement with care, justice, custodial, residential care, or supported accommodation services.
